This session “Surgical Approaches of the TMJ — When to Recommend Each Technique?” provided a comprehensive overview of the various surgical access routes and procedures used in the management of temporomandibular joint (TMJ) disorders, with emphasis on clinical decision-making. The talk focused on the indications, advantages, limitations and rationale for selecting specific surgical techniques — from minimally invasive arthroscopic procedures to open joint approaches — in different clinical scenarios involving internal derangements, pain disorders, trauma sequelae and structural abnormalities of the TMJ. Attendees learned how to match patient pathology with appropriate surgical access and intervention, balancing efficacy, safety and functional outcomes.

LEARNING OBJECTIVES
By the end of this session, participants were expected to be able to:
List the major surgical approaches to the TMJ, including preauricular, endaural, postauricular and other access techniques, and understand their relative visibility, exposure and risks.
Describe indications for minimally invasive procedures such as TMJ arthroscopy (levels I–III) and arthrocentesis — and when these techniques are preferable over open approaches in early or less complex cases.
Explain when open surgical access is appropriate, including procedures like disc repositioning, eminectomy, condylotomy, or reconstruction, based on pathology severity and anatomical requirements.
Match clinical scenarios with surgical technique selection, integrating pathology presentation, imaging findings and functional goals to recommend the most appropriate approach for individual patients.
Discuss potential complications and pitfalls of different TMJ surgical approaches, including facial nerve risk and aesthetic considerations, and strategies to mitigate them.
ABOUT THE SPEAKER
Dr. Pastore is a Brazilian oral and maxillofacial surgeon with extensive training and international experience in TMJ surgery, arthroscopy, and complex TMJ pathology. He graduated in dentistry and holds a PhD from Universidade Estadual de Campinas (UNICAMP), with specialist and residency training at prestigious institutions including the Hospital das Clínicas (USP) and international fellowships. He serves as Coordinator of TMJ and Craniofacial Surgery programs and is faculty at institutions such as the Hospital Sírio-Libanês (São Paulo) and fellowship programs affiliated with AO CMF. His clinical expertise spans arthroscopic procedures, open joint surgery, anatomical dissection, and advanced TMJ management.
