Positive tumour margins following ablative surgery for head and neck cancer are associated with increased morbidity and mortality. Existing tools to assist intraoperative margin detection, such as frozen section, offer poor diagnostic performance and are not universally adopted. Specialised mass spectrometry techniques can analyse the plume of smoke generated by electrodiathermy (Bovie) or CO2 laser dissection and accurately distinguish tumour from normal tissue: this has potential for intraoperative margin assessment.

By the end of this session, participants will be able to:
Understand the clinical implications of positive tumour margins in head and neck cancer surgery, including associated morbidity and mortality.
Recognize the limitations of current intraoperative margin assessment tools, such as frozen section analysis.
Describe the principles of specialized mass spectrometry techniques for analyzing surgical plume from electrodiathermy or CO₂ laser dissection.
Evaluate how these techniques can differentiate tumour tissue from normal tissue intraoperatively.
Appreciate the potential of plume-based mass spectrometry for improving intraoperative margin assessment and patient outcomes.
