Bilateral Sagittal Split Osteotomy (BSSO) remains the cornerstone of mandibular orthognathic surgery, yet surgeons continue to seek refinements that improve split predictability, reduce unfavorable fractures, and minimize neurosensory complications.
Among the most discussed modifications in recent years is the infralingular osteotomy, which relocates the medial cut below the lingula. Advocates suggest that this approach may facilitate more favorable splitting patterns and reduce technical challenges associated with conventional BSSO. At the same time, questions remain regarding its effects on inferior alveolar nerve position, neurosensory outcomes, and long-term clinical advantages.
In this lecture, Dr. Bobby John will present the rationale behind infralingular osteotomy, review the available evidence, and share his clinical experience with the technique. Through surgical videos, case presentations, and discussion of the current literature, participants will gain insight into both the potential benefits and the ongoing controversies surrounding this evolving approach.
Rather than presenting a settled answer, this session will examine whether infralingular osteotomy represents a genuine advance in orthognathic surgery—or whether important questions remain to be resolved.
SPEAKER
Dr. Bobby John MDS, MOMSRCS (Edinburgh), MFDS RCPS (Glasgow), FIBCSOMS, FDSRCS, FIBOMS is an Associate Professor in the Department of Oral and Maxillofacial Surgery at Government Dental College Kottayam. He completed both his undergraduate and postgraduate dental education at Government Dental College, Calicut, India. He holds Membership and Fellowship qualifications from the Royal Colleges of Surgeons in Edinburgh and Glasgow and is a Fellow of both the Indian and International Boards of Oral and Maxillofacial Surgery. He has received advanced training in TMJ arthroscopy and temporomandibular joint replacement surgery. Dr. John is actively involved in clinical research, surgical education, orthognathic surgery, TMJ surgery, and maxillofacial reconstruction. He has published peer-reviewed work on orthognathic surgery techniques, including comparative studies evaluating infralingular versus supralingular medial osteotomy in BSSO. He currently serves as Associate Professor in the Department of Oral and Maxillofacial Surgery at Government Dental College, Kottayam, Kerala, India.
