LEARNING OBJECTIVES

By the end of this session, participants were expected to be able to:

  1. Understand the clinical indications for total TMJ replacement, including end-stage degenerative disease, severe ankylosis, trauma, and failed prior surgeries, and how to identify appropriate candidates for surgery.

  2. Describe the surgical planning process, including preoperative imaging, prosthetic design considerations, and the use of emerging technologies (e.g., virtual surgical planning and customized implants) that enhance precision and fit.

  3. Discuss technical aspects of the procedure, such as access approaches, component placement, soft-tissue management, and intraoperative decision-making to maximize function and minimize complications.

  4. Interpret outcomes and complications from a large clinical cohort (250 cases), including functional results, pain relief, mandibular mobility gains, postoperative care, and strategies for preventing and managing adverse events based on long-term experience.

  5. Integrate evidence-based principles into practice, linking current literature on TMJ total joint replacement with real-world surgical experience to improve patient care and surgical success in complex reconstructive cases.

ABOUT THE SPEAKER

Dr. Silva is a Professor of Oral and Maxillofacial Surgery at the University of California, San Francisco (UCSF), and serves as Chief of Dental Service and full-time Oral and Maxillofacial Surgeon at the San Francisco Veterans Administration Health Care System, where she leads management of complex maxillofacial cases. She has co-authored and co-edited multiple textbooks and publications on TMJ disorders and reconstruction, including comprehensive surgical approaches and the role of total joint replacement in advanced cases. Her clinical and academic work focuses on TMJ reconstruction, surgical simulation, virtual planning, and multidisciplinary care, and she has presented extensively at national and international meetings
