In this talk, Neil Brierley provides a comprehensive overview of palatal fistulae, their classification, and surgical management. Palatal fistulae are a challenging complication following cleft palate repair, often leading to speech issues, nasal regurgitation, and patient discomfort.
Brierley discusses the different classification systems for palatal fistulae based on size, location, and etiology, helping surgeons determine the most appropriate treatment strategy. He highlights key surgical approaches, including local flap techniques, regional tissue advancement, and the role of secondary bone grafting when necessary. Additionally, he addresses factors influencing successful closure, such as tissue quality, prior scarring, and patient-specific anatomical considerations.
Emphasizing a multidisciplinary approach, Brierley explores the importance of timing in surgical intervention and adjunctive therapies, such as speech therapy, to optimize functional outcomes. The talk provides valuable insights for surgeons dealing with these complex cases, focusing on both technical refinements and patient-centered care.
